Become An Invacare Provider Today
How Can I Become an Invacare Provider? To become an Invacare provider, follow these easy steps:
1. Download and print out the Invacare Provider Application.
2. Fill out the application. All sections of the form must be filled out completely.
3. Upon completion of your application, we will notify your local Invacare territory business manager (TBM) to contact you and review your application for processing.
The following items must be completed before you can become an Invacare provider:
1. A territory business manager has reviewed and approved your application
2. You have submitted your completed credit application
3. A storefront location has been obtained
4. A $1,000.00 minimum opening order has submitted
5. Your state accepted resale certificate or tax exemption certificate has been sent to Invacare
Please note that your application cannot be processed until your local territory business manager (TBM) has signed your application, obtained an opening order, and validated that all other required items are completed. To obtain your local territory business manager's contact information, contact the CRM Group at 800-221-1559 x2238. Once your local TBM has provided us with all required items, your application will be reviewed and processed within 3 business days.
